APPLICATIONS ARE AGAIN BEING TAKEN!!
The cost of adding a name to the Monument is now $100.00
To add a name to the Monument, take a few minutes and look over the application. If you are unsure of the dates of service, or exactly what stone the name should be put on, leave this blank. Most importantly, carefully spell out the name that you want included and what time period the veteran served. Be sure to fill out the contact information or the application will be delayed. This information is especially useful if we need to contact you for any reason.
Please click the link below and it will take you to a page where you can print an application. Print out and sent the application to the address near the bottom of the application.
The application is very straight forward, and can be completed in a very short time.
Remember, if you do not have all of the information that is requested on the form, please write down what you do have available.
Whenever possible, the committee will try their best to help with any missing information and will contact you by email, telephone or letter to confirm the information is to your satisfaction.
This is very important!! We will do everything possible to ensure the information on the Monument is accurate, but we will rely on you as the primary source of information. This means that we will spell names and dates on the Monument exactly as they appear on the application. Please take your time to get everything correct.
Just click the 'application' button below to go to the application and print from your computer. To fill in the form from your computer, just place your cursor over the appropriate line and start typing. When you are finished just print out the form. Send both the application and your payment to the address that is on the application.
Names may be submitted anytime. They will be engraved on the monument once or twice a year provided there are enough names.

Memorial Day - April 15th
Veterans Day - October 15th
